Travel with infant to Italy

I will primarily be using buses and trains to get around but may need an cab a few times. I don't know if I need my car seat, or not. I was in Italy many years ago and from what I recall most cabs didn't have seat belts. Also, I don't know how I'll get around all day with a car seat on my back. Any advice?
